Nurse's Notes
Please allow me to introduce myself and to welcome you and your child to John Dickinson High School. If you child has any specific health needs or concerns please feel free to contact me at your earliest convenience.
Some points of interest to insure that your child receives quality medical care in our Health Suite:
- All immunizations and physicals must be up to date. Please provide any updated information regarding any immunizations that your child may have received since last year.
- Please fill out Emergency Cards in as much detail as possible. List all possible numbers to reach you in the order in which you like those numbers to be called. Be sure to sign the card and list any allergies, medical conditions and permission to administer Tylenol if you would like for me to do so.
- Please inform me of any medical and /or food allergies, any medical conditions or special needs that your child may have so that we may work together to formulate your child’s individual health care plan. Please send in any necessary equipment that your child may need and be sure that it is clearly labeled.
- It is my policy to contact you regarding visits that your child makes to the Health Suite other than routine visits for general problems such as headaches, minor injuries, etc. If you have any concerns at all, please feel free to contact me at 992-5504.
- Medications cannot be administered without clear written current instructions from a physician and a signed consent form from you. Medication must be in labeled pharmacy containers. Ask your doctor and or/pharmacist for an additional labeled prescription container to be kept at school. These are State of Delaware regulations and no exceptions will be made. In the event that your child uses an inhaler, and you would prefer that he /she carry the inhaler with them, you will need a physician’s note giving them permission to carry it with them in addition to the information required above. No exceptions will be made.

- Sometimes our child ask to come to school when they are not feeling well so that they may take a test or participate in school activities. Although we admire their enthusiasm, we would prefer that they not come and possibly transmit their illness to someone else. Please do not send them to school if they indicate that they are ill in the morning prior to coming to school
It is my goal to keep our children as healthy as possible at John Dickinson High School. It is of the utmost importance to streamline each child’s healthcare to insure that they spend the majority of their day in the classroom learning.
